CHADIS Suggested Questionnaire Use
While CHADIS offers a number of different questionnaires, screens, and measures, some of these are more appropriate to assign on a regular basis. These questionnaires are listed here with their suggested schedules.
Offices that are new to CHADIS will automatically receive this suggested schedule as their "automatic assignment" template, which can be modified at any time. Please note that automatic assignments only occur when a patient is first created.

Well-Child Visits
These are questionnaires suggested for all routine, well-child visits.
