Patient Health Questionnaire for Adolescents (PHQ-A) Release Announcement

Dear CHADIS Customer,

We are pleased to announce the availability of a new CHADIS questionnaire, the Patient Health Questionnaire for Adolescents (PHQ-A; Johnson et al., 2002). The PHQ-A is an 83-item, self-report questionnaire that is intended to be completed by the adolescent. It was designed for the purpose of assessing anxiety, mood, eating, and substance use disorders among adolescent primary care patients. The PHQ-A was created to obtain data in a few minutes of the patient's time that may be used to determine whether or not it is probable that an adolescent patient currently has an anxiety, mood, eating, or substance use disorder. PHQ-A diagnoses are most appropriately regarded as provisional diagnoses that require confirmation through further examination of the patient by the physician or a qualified mental health professional.

In order to use this questionnaire, you must first create a new respondent for the patient, AFTER the parent has created her login and the patient in CHADIS. You can add respondents from the patient detail screen by clicking on 'add new' at the bottom of the respondent section. Add the adolescent as the respondent by filling in his first and last name and selecting 'self' in the relationship drop down box. CHADIS will generate the username and password for the patient, which you can give to him via email, fax or phone. For more information on this process, go to creating a new patient in the demo instructions and refer to step #7 for how to create an additional respondent. Once you have added the respondent, you can manually assign the questionnaire to the patient. For more information on this process, visit the demonstration instructions on our website.

You can check out the growing family of pre-visit questionnaires now available through CHADIS on our website (www.childhealthcare.org).

Thanks,

The CHADIS Team
